Coverage Details
Plan Details and Terms
12-Month Term
The plan begins on the enrollment date after payment in full and expires 12 months later. There is no automatic renewal. Active renewal and payment are required for another 12-month term.
Payment
The 12-month plan fee is due in full at enrollment. Treatment payments are due when services are provided.
New Patients Welcome
Ask the team about enrolling after the new patient special. Plan benefits begin upon enrollment after payment in full and do not apply retroactively. Offers cannot be combined.
Renewal
Fees, benefits and terms for a future 12-month term will be provided before renewal.
Complete Plan Terms
Patients should review the plan agreement for all exclusions, limitations, refund policies and eligibility requirements before enrolling.
